Infertility in women and men, its causes and treatment
15% of spouses face problems in not having children in the first year of marriage, and not having children does not necessarily mean infertility, because infertility in medical terms is the permanent state with which there is no hope of having children.

40% of these cases are related to women, 40% are related to men, and there are between 15-20% of cases the causes are unknown, and not specified.

Dr.. Nicola Farah, a specialist in gynecology and infertility problems, and the director of the maternity department at the Holy Family Hospital, spoke to "Your Doctor" about the problems of infertility in men and women, their causes, types, and ways of treatment in this interview.

What are the causes of male infertility?

The causes of male infertility are due to either congenital causes, or acquired causes.

Congenital causes are congenital malformations, diseases, or incompleteness of the reproductive system, or they may have genetic factors and problems with male chromosomes. As for the acquired causes, it is due to an imbalance of hormones in the functions of the inner glands, or in the channels responsible for transferring the "implant" from the testicles to the case of ejaculation, or every other disease that affects the testicles or urinary tract, which are the positions of "production" and "ejaculation" of the sperm, It can cause infertility in men.

Most cases of infertility in men the causes of which are unknown, so the treatment of infertility or infertility in men is more difficult and more complex than in women. There is a very important issue that can affect very sharply the non-procreation of a man who is known as "Abu Zemmour", which is a viral infection that affects the salivary glands, and this virus has tendencies to infect the testicles during childhood, and it is able to destroy all cells that produce sperm .

Other causes that may be the result of diabetes, high blood pressure, or other diseases that may affect sperm production, such as infections of the testicles, prostate inflammation, or urinary tract that usually affects a person from multiple and unattended sexual relations, and in recent years we have witnessed other related causes In the environment or in the nature of work, such as men who work at high temperatures in asphalt and paving the streets, or peasants who use types of fertilizers that may hinder the production of sperm, over time, as well as occupations that require the worker to sit for long periods such as truck and taxi drivers, not By chance, the testes are located outside the body, because they need a lower temperature than the body temperature. And when the temperature rises, it may affect the "implant" production.

Riding bicycles Does it increase pressure on the testicles and consequently sterility?

There is no reliable rule in this regard. It often depends on the individual’s body and its response to the surrounding environment. If we say, for example, that smoking affects sperm production, we may find a smoked man with ten children !! It is relatively and individually more than an equation or a fixed base.

Add to that cases of hernia or cancerous diseases that may affect childbearing, as well as psychological conditions and pressures that take a large part of research today, which may lead to non-erection or to premature ejaculation or affect the production of sperm.

Doing a lot of masturbation in adolescence will lead to sexual dysfunction in the future?

No ... masturbation is a sexual process that one person performs without his partner, and it does not cause sterility or impotence, but it is likely that it causes what is known as rapid ejaculation in practice, due to excessive irritation, and this is not scientifically confirmed.

Does the lack of sexual experiences before marriage affect relations after marriage?

This is certain in conservative societies, the shift from a single life to a married life forms, i.e. from the situation in which the matter is prohibited, to the situation in which the matter becomes totally available affecting the girl in particular and witnessing changes in the menstrual cycle system, accompanied by physical and psychological changes, and because of the lack of Previous experience, it may find in the sexual relationship an automatic process that is not a process stemming from a feeling of love, longing, tendencies and desire, so that such a relationship with the passage of time the woman no longer finds any pleasure in it, while the man turns to other sources that give him more pleasure and contentment that he misses, Especially if he begins to question himself and his ability to do this job with his wife.

Women's infertility problems

The psychological aspect is one of the main causes of infertility in a woman. The woman’s body is very sensitive to changes, and if she is exposed to a shock as a result of a certain event, or a change in food patterns, or a change in the weather, she finds all that affects the woman’s menstrual cycle, and this change is accompanied by a change in Hormones and production of mature eggs, ready for fertilization.

As for the organic reasons, we know that the menstrual cycle is subject to the work of the pituitary gland and the brain. The menstrual cycle begins with orders and hormones from the brain, to the pituitary gland that in turn prepares the ovary, which produces the egg and prepares the uterus to receive the fertilized egg. Any defect occurring in any of these elements or organs would disturb the menstrual cycle and thus not produce an egg ready for vaccination.

How can the defect be located?

There are cases in which the pituitary gland produces an excess of the prolactin milk hormone, which is the first thing that we examine in a woman, and then we examine the hormone produced by the ovary, so if we find a defect in one of these two tests, we can guess that the problem is in the pituitary gland, at the stage More advanced, we examine the ovary, which receives orders from the pituitary gland, and then we turn to the possibility that there is a cyst in the ovary, which is an ovary that contains a large number of encapsulated eggs (inside bags), but these eggs do not mature to the extent that they can produce an egg Mature and pollinated.

Other problems that may be caused by fallopian tubes or what is known colloquially (tubes) that may be closed due to certain infections, or the presence of fluid bags that press these channels, preventing eggs from passing through them, as well as congenital anomalies in the uterus, reproductive system, ovary, and other times the deformation of the vagina Such as adhesion and obstruction that impede the access of animals to the ovary and its union with the mature egg.

Are there treatments for all of these cases?

As for the dysfunction of the pituitary gland, it can be treated with medication, which is multiple and treatment is according to the deficiency in the type of hormone produced by the gland, including drugs or injections that directly affect the ovary, and in recent years this field (treatment of infertility) has achieved a huge qualitative leap.

Or in cases where there is a blockage in the fallopian tubes, the treatment is usually to resort to IVF (tube). Vaccination is also used in cases where the number of sperms in a man is less than the average, or a large percentage of these animals are deformed or their movement is weak. Then we resort to the process of improving and injecting these sperm into the ovaries.

When does a newlywed woman resort to treatment if the work does not happen?

After a year of trying to conceive naturally, if the couple did not succeed, they should go for treatment, and I do not concede the need for the husband to undergo examinations as well, except in exceptional cases and the appearance of indicators or abnormal signs such as the appearance of thick hair on the woman's body, for example, then this woman produces hormones Remembrance. Or in cases where the menstrual cycle is every two or three months, or a woman cannot tolerate sexual activity, all of these are abnormal indicators that need to consult a doctor in advanced stages without waiting.

What happens as a woman gets older?

Over the age of 35 a woman's egg quality decreases, but the man weakens her ability or desire to have sexual intercourse

At what age is pregnancy considered dangerous?

Over the age of 35 we recommend that additional tests be performed for a pregnant woman, and according to the regulations of the Ministry of Health over the age of 37, every pregnant woman must perform a placenta water examination. In addition to tests for diabetes, liver and kidneys.

Contraceptive pills do infertility?

If a woman has a regular menstrual cycle before taking birth control pills, these drugs do not cause infertility. Sometimes, these drugs solve the problems of infertility, and in many cases we recommend that women take birth control pills in order to regulate the menstrual cycle, and pregnancy often occurs after stopping taking these pills or drugs.

Misconceptions and social beliefs!

There are many misconceptions shared by women, especially the elderly, while the real mistake was taking place in their time when women were resorting to treatment for elderly women treated with herbs and folk medicine for their inability to reach a specialist doctor. One of these misconceptions we mentioned mention birth control pills and their relationship to infertility, and it is true that the contraceptive pill at all does not cause sterility. The belief that subjecting a man to a severe blow in the back may affect childbearing ... This is a misconception. The belief that the "Epidural" needle causes paralysis, sterility, or back pain in the future is not true. The belief that IUDs should not be used as a method of contraception after a cesarean is incorrect.

Women who abort a pregnancy and perform an endometrium abstraction, does this affect pregnancy in the future?

This process does not necessarily affect. If the abstraction process, which is usually performed by something like a spoon, is done correctly, then the uterine cells regenerate on their own, but if done deep, they may harm the lining of the uterus and the tissues responsible for the regeneration of the endometrium cells, then there will no longer be Uterine cell production, and intrauterine adhesions may occur. We prefer the suction process over abstraction.
